Privacy Policy

This notice describes how patient medical information may be used, disclosed and accessed. Please review carefully.

Palmetto Surgery Center's medical staff, practitioners and non-practitioners, who provide services to patients in this facility, are committed to making every reasonable effort to maintain the privacy of a patient’s PHI (protected health information). We may use and/or share this information as follows:

• FOR TREATMENT [example: the patient’s medical team]
• FOR PAYMENT [example: insurance providers]
• FOR HEALTHCARE OPERATIONS [example: scheduling]

Other less common purposes which permit or require PSC to release PHI include:

• Workman's Compensation [example: work related injury]
• Law Enforcement [example: abuse report]
• Health Department [example: infectious disease report]
• Legal Proceedings

Other use or release of information will be made only with the patient’s written consent which can be changed at any time.

The HIPAA Privacy Rule gives patients the following rights regarding PHI:

• The right to request a restriction on certain uses and disclosures of information.
• The right to obtain a paper copy of PSC Privacy Notice.
• The right to inspect and obtain a copy of the medical record.
• The right to add to or correct the medical record.
• The right to receive a record of disclosure of the PHI.

The HIPAA Privacy Rule requires that Palmetto Surgery Center:

• Maintain the privacy of every patient's PHI.
• Describe PSC duties and practices regarding PHI.
• Abide by the terms of the Privacy Notice currently in effect.
• Respond to all reasonable patient requests about the PHI.

An individual may complain to the Surgery Center and DHEC if it is believed privacy rights have been violated.
